Documentation AI in a clinical setting has one honest design: the clinician signs everything. Suithera enforces it in code.
A generated note lives outside the chart until you review and sign it. Unsigned drafts expire; they never leak into the record.
AI documentation runs only for clients with recorded consent. A missing gate blocks the call — and the block itself is audit-logged.
Clients are identified by initials at intake. Note content is encrypted at rest and in transit, with an audit entry on every read.

Does the AI ever write directly into the chart?
No. Every output is a draft in a separate review state. It enters the chart only when you sign it — and the signature event itself is logged.
What does the model actually see?
The structured session summary you enter — initials, duration, presenting issue, interventions, response. Note content is encrypted at rest; prompts are stored in the audit log as hashes, never as text.
What if a client declines AI documentation?
You write that client's notes manually, exactly as before. The consent gate blocks AI calls for them, and any blocked attempt is recorded in the audit log.
Can I keep my own note templates?
Yes — templates are yours: section order, required fields, and phrasing. The AI drafts into your structure rather than imposing one.
